In West Belfast Federation, funding has been provided by the DoH for five full-time General Practice Nurses (GPN). The nurses work closely with Practice teams, including Practice employed nurses, within which they are based.
Federation General Practice Nurses provide nursing services to the practice population through evidence based, quality focused care. They undertake a range of nursing assessments and provide appropriate care/treatment in conjunction with GPs, Advanced Nurse Practitioners, Senior General Practice Nurses, Treatment Room Nurses, Healthcare Assistants and other members of the Multidisciplinary Team.
GPNs are involved with other team members in identifying practice population needs with a specific focus on management of Long-Term Conditions.
They provide nurse-led clinics for a range of long-term conditions and public health activities including Immunisations, Diabetes; Asthma & Chronic Obstructive Pulmonary Disease; Cardiovascular Disease; Chronic Kidney Disease; and Hypertension.
Communicating effectively with patients, families/carers and ensuring they are involved in decisions about their care/treatment, GPNs support patients and carers with a range of conditions including brief focused lifestyle advice to enable self-management.
